Job Vacancy: Wellbeing Facilitator (Part Time)

We have an exciting opportunity for you to join our Caregivers Connected Service in Gateshead and play a key role in shaping a bespoke and engaging service that supports and positively impacts caregivers.
Support unpaid caregivers to identify and deliver appropriate support mechanisms to meet the identified needs of the individual or family through internal or external support, organisations and statutory agencies.


Wellbeing Facilitator

Hours: 21 hours per week

Type: Permanent

Salary: Band 1b, Starting Actual Salary £14,107 rising to £14,409 following successful 6-month probation.

Location: Caregivers Connected Gateshead, Aidan House, Sunderland Road, Gateshead, NE8 3HU


The successful applicant will:

  • Support and empower unpaid carergivers, including parent carergivers, to build confidence and resilience.
  • Provide support and information via face-to-face, telephone, and other channels.
  • Facilitate and run carer groups across Gateshead.
  • Complete statutory carer assessments (MOSAIC), light-touch assessments and monitoring requirements (training will be provided).
  • Provide condition-specific information and signpost unpaid caregivers to community services, respite, and other agencies.
  • Support young adult and adult caregivers with transitions into adult services, education, employment, or training.
  • Attend safeguarding meetings and adhere to local and national adult and child safeguarding policies.
  • Work with external stakeholders across Gateshead, including GP practices, Health & Social Care, and community partners.
  • Manage caseloads and contribute to service planning through team meetings.
  • Maintain accurate records, database entries, correspondence, and facilitate feedback from assessments and closed cases.
  • Make use of supervision, training, and peer support.
  • Travel/commute around Gateshead.

Desirable Criteria:

  • Access to own vehicle.
  • Experience using MOSAIC.
  • NVQ Level 2 in Health and Social Care (full or part qualified) and/or 1–2 years caregiver support service experience.

  • Ability to engage non-judgmentally with caregivers from diverse backgrounds, cultures, ethnicities, ages, and abilities.
  • Resilient and confident in handling challenging or distressing situations.
  • Able to manage own workload effectively.
  • Collaboratively work within a team.
  • Competent with Microsoft Office application.
